Contact the Metabolic-Surgical Weight Management department at 303-861-3400 to confirm if you have the bariatric surgery benefit and request to have your medical chart reviewed to see if you are medically eligible for weight loss surgery. Prior to surgery, all patients must undergo a thorough evaluation, including a pre-operative psychology screening and a pre-operative medical and surgical screening.
